Tameside Multi-agency referral form

The Tameside Multi-agency referral form can be used by practitioners to make referrals to Children's Social Care only. It is not a substitute for carrying out a Common Assessment (CAF) but the CAF can be used as supporting information to the referral. If the referral is urgent then making a phone call to Children's Social Care is a better option but the referral form should be sent to Children's Social Care as soon as possible after making the phone call.
